How to request payment from the Devon PTO

You have two options:

  • Complete online form and file your receipts electronically
  • Print out hard copy of form and submit by hand/mail

Print out hard copy of form and submit by hand/mail

  1. Complete the “Devon PTO Check Request Form.” A copy is attached to this email. It can also be found at devonpto.org under “PTO Business.”
  2. Attach the original receipts to the check request form.
  3. Return the completed form and receipts to the PTO Treasurer (Cathy Rains):
    1. Drop off at the school –
      I generally check the school mailbox at least once per week. However, if I need to process the request as quickly as possible, please send me an email to let me know it’s there.
    2. Mail or drop off in my mailbox at the my house
      247 Berkeley Rd
      Devon, PA 19333

Please allow extra processing time for checks over $500 since a second signature is required.

Side note:  If ordering on Amazon, don’t forget to go through the Amazon link on www.devonpto.org so the school will get a percentage of the sale!
